Spherix Global Insights Research Reveals a CKD Symptom Disconnect: 86% of Patients Report Symptoms While Nephrologists Classify Nearly Half as Asymptomatic

New Patient Voice Dynamix™ research finds substantial fatigue, emotional and quality-of-life burdens as patients seek greater treatment choice and guidance on what to expect as CKD progresses.

Exton, PA, Sept. 11, 2026 (GLOBE NEWSWIRE) -- Chronic kidney disease (CKD) is often described as a silent condition, but new research suggests the patient experience may be considerably less silent than clinical assessments imply. Findings from Spherix Global Insights’ Patient Voice Dynamix™: Chronic Kidney Disease, 2026 reveal a marked disconnect between patient-reported symptoms and how disease burden is characterized in physician-reported patient charts.

Patient-Reported Symptoms Reveal a Less ‘Silent’ CKD Experience

While nephrologists classified nearly half of stage 3–5 CKD patients captured in Spherix’s latest Patient Chart Dynamix™: Chronic Kidney Disease (US) research as asymptomatic, only 14% of patients surveyed through Patient Voice Dynamix™ reported experiencing no symptoms. Fatigue was reported by nearly two-thirds of patients, and anxiety or depression by almost half. One of the largest differences emerged around flank pain: one in five patients reported experiencing it, compared with nephrologist reports of flank pain among just 2% of patients captured in the physician chart research.

The burden extends beyond physical symptoms. More than half of surveyed patients rated their quality of life as moderate or poor. About two-thirds said physical health caused them to accomplish less than they would have liked during the prior four weeks, while nearly three in five said the same about the impact of emotional health. Black and Hispanic patients reported a lower average quality-of-life than by White patients.

As one participant explained, “People can look physically well while struggling with the condition internally.” The comment underscores a central theme in the findings: aspects of CKD burden that may not be readily visible during a clinical encounter can nevertheless have a meaningful impact on patients’ daily lives.

Treatment discussions also leave a meaningful share of patients without a strong sense of agency. More than one-third said no treatment options were discussed at their most recent visit. One in six reported never having discussed treatment options with their kidney doctor, while a similar proportion felt they had little or no choice in their treatment plan. Among patients who did discuss treatment, Black patients reported the lowest perceived choice, significantly lower than White patients.

Financial Burden Persists Despite Broad Insurance Coverage

Despite nearly universal insurance coverage in the sample, more than one-third reported at least some medical debt, and more than one-quarter said they delay appointments because of cost. Financial-support needs also differ across patient groups: Black patients sought information about financial-assistance programs at twice the rate reported by White patients.

Together, the findings suggest that improving the CKD patient experience may require looking beyond traditional measures of clinical disease burden. More systematic symptom check-ins, shared treatment discussions, forward-looking disease education, and greater recognition of patients’ emotional and financial experiences may help align care more closely with the realities patients report living with every day.

Patient Voice Dynamix™: Chronic Kidney Disease, 2026 is based on a 30-minute online survey of 367 U.S. adults ages 21–81 who had been diagnosed with CKD, were not on dialysis and had not received a kidney transplant. The quantitative study was fielded March 6–June 1, 2026, and was supplemented by eight qualitative interviews conducted February 16–April 6, 2026. The physician comparison is from Spherix’s 2025 Patient Chart Dynamix™: CKD Non-Dialysis study of 1,052 stage 3–5 CKD patient charts; the two samples are separate and should be interpreted as a cross-study comparison.
